The Challenge
Over 50% of families in ZIP codes 00923 and 00924 live below the poverty line. Nearly 30% of households are led by single mothers. There are decades of distrust toward institutional systems because services have been fragmented, bureaucratic, and disconnected from each other. We're changing that.
How It Works
Family Identification
We meet you where you are. We work in 7 schools in your area (grades K–3) to identify families that would benefit from our support.
Needs Assessment
We look at 8 key dimensions of family well-being: health, education, income, employment, housing, transportation, childcare, and food security. Not to judge — to understand how to help.
Family Action Plan
Together we create an annual plan with specific goals for the adults and children in your family. You decide the priorities.
Case Management & Referrals
Your Navigator walks with you continuously: removing barriers, connecting you to services, and making sure no one falls behind.

Individualized Case Management
A dedicated Family Navigator who works with you and your family directly. Co-created plans, continuous monitoring, and comprehensive support year-round.
On-Demand Navigation
No appointment needed. When you need something specific — help with a form, a referral, guidance about a service — just reach out and we'll assist you.
